Problem: NHS Birmingham East and North (BEN) have one of the highest hospital admission rates for heart failure in the West Midlands and a full needs assessment of services was needed to fully understand the services currently offered by the PCT to patients, in primary, community and secondary care
Solution: Population Health Manager was used to conduct a range of analysis included in the Needs Assessment. The trust was keen to understand the spread of heart failure prevalence across the population.
Outcome: The trust has focussed on the development of community services across the PCT, moving services into primary care to enhance both the service and experience for the patient and financial cost. Improved education and titration in addition to providing much greater support to GPs have been key focuses for the Heart Failure team at NHS Birmingham East and North.
