Dr Foster on BBC Radio 4's The Report 26 May 2009

Influential BBC Radio 4 programme, The Report, examined the fall-out from the Healthcare Commission’s report into the Mid Staffordshire Hospital affair. Investigative journalist Simon Cox explores whether it's true that Mid Staffordshire was an isolated case.

Roger Taylor, Director of Product Strategy and Design for Dr Foster provided clear analysis of the HSMR (Hospital Standardised Mortality Rates) alerts sent to Mid Staffs by the Dr Foster Unit at Imperial College London. He explained how the alerts work on a monthly basis and the usefulness of indicators to identifying potential problems. The programme also featured Sir Brian Jarman, Director of the Unit and originator of the methodology behind HSMRs.

A government-commissioned inquiry is to deliver its verdict on what led to the catastrophic failures at Mid Staffordshire Hospital. The Health Secretary has said he is confident that Mid Staffordshire is an isolated case.

Simon Cox investigates whether that's true. He asks why the failings at Stafford weren't picked up by a host of organisations and whether other hospitals should also be causing concern.
