London

  • 1. Tower Hamlets reduced A&E by 6.4% after we involved the community in the solution
  • 2. Lambeth PCT saw smoking cessation rates increase by 110% as a result of our social marketing campaign
  • 3. DFI’s work with Redbridge PCT on a stop smoking campaign helped the Trust meet their annual targets, featured on GMTV’s local London programme and on LBC radio news and is being considered as a template for work in other areas of care
  • 4. Ealing PCT used PET to gather feedback from 2,600 patients and members of the public in the first six months of their engagement programme
  • 5. Sutton and Merton PCT used social marketing techniques to better understand community needs and more appropriately redesign smoking cessation services
  • 6. Hammersmith Hospitals NHS Trust have strengthened their governance committees and have improved their analysis and reporting with RTM and PPM
  • 7. St Mary’s NHS Trust now has one of the highest survival rates in the country for the treatment of acute heart attacks after improving their working practices and data collection with PPM and RTM

  • 10. Chelsea and Westminster Hospital NHS Foundation Trust used PPM to reduce length of stay and increase day-case rates
  • 11. Redbridge PCT are using HUM and PPM to get a better understanding of local population health needs and to track and manage patient care
  • 12. In Ealing we have identified that 58% of reception school children in the Mosaic lifestyle type ‘Asian Enterprise’ are overweight or show some degree of malnutrition
  • 13. Westminster PCT used the Intelligent Commissioning Board report to structure board meetings and plan non-executive board member inductions
  • 14. Homerton University Hospitals NHS Foundation Trust are using PET to hit their target of a 75% response rate from patients using their services
  • 15. The PCTs in North East London decided to redesign their breast screening services after DFI ran a series of research forums and market analyses to help understand community needs
  • 16. Brent Teaching PCT used teenage volunteers to distribute sexual health messages after analysing teenage lifestyles and health behaviours at postcode level to identify which areas to target
  • 17. Hounslow PCT used PPM to identify ways to reduce the £7m cost of A&E attendances, including placing a nurse practitioner in the department to greet, and refer, patients if possible

  • 18. Bournemouth and Poole PCT used our Intelligent Commissioning Board report to help structure their new, stream-lined governance procedures as part of the reconfiguration of PCTs
  • 19. The Royal Bournemouth and Christchurch Hospitals NHS Foundation Trust reduced their mortality by 14% and increased accountability across their clinical teams
  • 20. Torbay Care Trust uses PPM to better manage information, resulting in improvements such as reductions in unnecessary emergency admissions

  • 21. West Suffolk NHS Hospitals NHS Trust saved £1million after making changes to its discharge procedures
  • 22. Colchester Hospital University NHS Foundation Trust lowered their mortality rate by 18% after creating a new surgical rota
  • 23. Milton Keynes PCT used the Intelligent Commissioning Board report to put together a broad range of performance indictors, leading to more strategic and effective board reports
  • 24. Norfolk and Norwich University Hospital NHS Trust have reduced their readmission rate for paediatric appendectomies by 10% by setting up a special aftercare programme
  • 25. Papworth Hospital NHS Foundation Trust was listed as one of the best trusts in the country by the Healthcare Commission for patient experience. They use PET.
  • 26. Wellesley Road Practice used PET to survey more than 560 patients in order to find out exactly what patients required when they requested longer opening hours

Yorkshire and The Humber

  • 27. Mid Yorkshire Hospitals NHS Trust reduced their mortality rate from 98 to 88 by routinely monitoring clinical outcomes
  • 28. A Barnsley PCT COPD information and awareness campaign resulted in more than 6,000 requests for information packs and 314 people asking for direct contact with smoking cessation service advisors
  • 29. United Lincolnshire Hospitals NHS Trust tracks patients through their recovery pathway and their readmission rate is now well below the national average
  • 30. Community matrons in Ashfield and Mansfield use HUM to identify and manage their patients with chronic conditions, resulting in fewer emergency admissions
  • 31. In Humber, teenagers receiving information on sexual health via Bluetooth passed the message on to an average of five friends
  • 32. Nottingham City Hospital uses PPM to increase teamwork and improve data management, already resulting in reductions in lengths of stay

North West

  • 33. East Lancashire PCT developed their dashboard style of board reporting from the guidelines set out in the Intelligent Commissioning Board
  • 34. We used a reputation audit to design a ‘brand benefit statement’ for the Liverpool Women’s NHS Foundation Trust, helping it to inspire staff and market itself as provider of choice and the authority for women’s health
  • 35. Salford Royal NHS Foundation Trust have improved their coding and information sharing across the trust after using RTM and PPM to discover that their length of stay was longer than average
  • 36. Royal Liverpool Children’s NHS Trust have increased their day case rates after comparing themselves to the national average and other similar trusts
  • 37. Sefton PCT worked with DFI to identify nine wards with high risk of COPD and home to heavy smokers, contacting more than 12,000 households as part of the information campaign
  • 38. A PCT-wide consortium in Warrington tasked each GP with identifying their top 10 most expensive patients so they could save time, money and resources by targeting the most relevant problems
  • 39. We contacted over 4,000 households in Cheshire & Merseyside as part of a COPD and smoking cessation campaign. 65% of the people contacted requested further information and support
  • 40. Eddie Brownlow decided to quit smoking and became involved in his local support group for people with breathing difficulties after our campaign reached him
  • 41. Liverpool PCT used to PPM to investigate rising A&E admissions and decided to implement a primary care unit in the department
  • 42. Public engagement in Western Cheshire showed only 50% were satisfied with out-of-hours services compared with 71% for A&E, thereby informing the redesign of urgent care around users’ needs
  • 43. We put more than 6,000 smokers in touch with stop smoking services in Greater Manchester, giving the Association of PCTs an estimated return on investment of £1,174,890 through avoiding 114 heart attacks and 83 strokes
  • 44. 800 people contacted the Oldham smoking cessation service after our Kick It campaign worked with Oldham FC to target young men from ethnic minority groups

West Midlands

  • 45. Walsall Hospitals NHS Trust reduced their mortality rate by 30% after using Real Time Monitoring to track their mortality ratio over four years
  • 46. Birmingham Women’s NHS Trust used DFI tools to build their case for Foundation Trust application and to implement a 100 Days communications plan
  • 47. Shropshire Country Commissioning Group found that PPM and RTM provided them with the necessary level of detail to analyse trends and improve patient pathways
  • 48. The Royal Wolverhampton Hospitals NHS Trust used RTM to explain an anomalous increase in fractured neck of femur mortality, proving their high clinical standards

East Midlands

  • 49. In Derby a total of 8,235 people signed up to increase their level of physical activity through a series of live events with even more signing up via the NHS Choices Derby microsite
  • 50. Former Erewash PCT and Ilkeston Community Hospital used PET to improve patient satisfaction with meals
  • 51. NHS East Midlands is working with DFI to develop an Information Intelligence System to better standardise and benchmark data

South Central

  • 52. Our stop smoking campaign for NHS South Central gained leads for over 18,000 potential quitters.
  • 53. GPs in Oxfordshire have joined together in locality groups and are using PPM to better manage data and provide more effective patient care.
  • 54. Our research showed that half of Slough’s diabetic population were unaware that they had the condition. Our Action Diabetes campaign took the screening services to the streets and led to a 164% increase in the early detection of diabetes

  • 55. Two-thirds of Brighton and Hove City’s PCT post-campaign survey respondents said that as a result of DFI’s binge-drinking communication and behavioural campaign, they would think more carefully about their drinking
  • 56. The Lighthouse Surgery identified 13 areas in which it could set up specialist clinics to help drive down costs and deliver care in the community
  • 57. The Central Eastbourne Consortium uses its Patient Forum to consult patients and has dropped the waiting time for ultrasounds by 14 weeks to four
  • 58. Thanet PBC Consortium have dropped their waiting times and re-referral rates after using PPM to support their commissioning and service redesign plans
  • 59. 87% of survey respondents in Brighton said they were likely to visit the Brighton “Sussed About Drink” website as a result of our communication campaign
